摘要:
目的观察散血明目片对糖尿病视网膜病变(diabetic retinopathy, DR)并发玻璃体积血( vitreous hemorrhage, VH)患者视网膜电图的影响?方法60例(85眼)DR并发VH患者随机数字表法分为两组,对照组30例(42眼)行玻璃体切除治疗,术后1mo玻璃体腔注射康柏西普,治疗组30例(43眼)在对照组的基础上于玻璃体切除手术后加用散血明目片口服治疗?治疗4mo后,观察两组患者最佳矫正视力( best corrected visual acuity,BCVA)?视网膜电图( electroretinogram, ERG) ?光学相干断层成像( Optical coherence tomography, OCT)检测两组患者黄斑中心凹厚度( central macular thickness, CMT) 情况?结果两组患者治疗前后不同时间BCVA差异有统计学意义(P<0.001),治疗后1mo两组患者BCVA相比,差异无统计学意义(P>0.05),治疗后2mo?3mo?4mo两组患者BCVA相比,差异均有统计学意义(P<0.05);两组患者治疗前后不同时间明适应a?b波振幅差异均有统计学意义(P<0.05),两组患者治疗前后不同时间明适应a波潜伏时差异均有统计学意义(P<0.05),治疗后3mo?4mo两组患者明适应b波振幅相比,差异均有统计学意义(P<0.05);两组患者治疗前后不同时间CMT差异有统计学意义(P<0.001),治疗后1mo两组患者CMT相比,差异无统计学意义(P>0.05),治疗后2mo?3mo?4mo两组患者CMT相比,差异均有统计学意义(P<0.05)?结论散血明目片联合康柏西普较单纯康柏西普治疗糖尿病视网膜病变并发玻璃体积血,能更好的减轻视网膜水肿,提高视力及明适应a?b波振,降低CMT?明适应a?b潜伏时?
Abstract:
Objective To observe the influence of Sanxue Mingmu Tablet on diabetic retinopathy ( DR) patients with vitreous hemorrhage ( VH) . Methods Sixty patients ( 85 eyes) with DR complicated with VH were divided into two groups by random number table method. 30 patients ( 42 eyes) in the control group received vitrectomy for treatment, 1MO intravitreal injection of Compaccip, and 30 patients ( 43 eyes) in the treatment group were treated by oral administration of Sanxue Mingmu tablets after vitrectomy on the basis of the control group. Several BCVA ( best corrected visual acuity) , electroretinogram ( ERG) , and OCT ( Optical coherence tomography) were observed to test the central macular thickness ( CMT) of the two groups after treatment of 4mo. Result The BCVA of patients in the two groups at different times before and after treatment showed statistically significant difference ( P < 0. 001) ; after treatment, the BCVA of patients in 1mo group showed no statistically significant difference ( P > 0. 05) ; after treatment, the BCVA of patients in 2mo group, 3mo group and 4mo group showed statistically significant difference ( P < 0. 05) . In the two groups before and after treatment in patients with different time, light adaptation of a and b wave amplitude difference had statistical significance ( P < 0. 05) , the two groups before and after treatment in patients with different time light adaptation, the wave of latent difference had statistical significance ( P < 0. 05) . After treatment 3mo and 4mo, compared two groups of patients b wave amplitude of light adaptation, differences were statistically significant ( P < 0. 05) ; There were statistically significant differences in CMT between the two groups at different times before and after treatment ( P < 0. 001) , no statistically significant differences in CMT between the two groups at 1mo after treatment ( P > 0. 05) , and there were statistically significant differences in CMT between the two groups at 2mo, 3mo and 4mo after treatment ( P < 0. 05) . Conclusion Compared with the treatment of diabetic retinopathy complicated by vitreous hematoceles with compaccip alone, Sanxue Mingmu tablet combined with compaccip can better reduce retinal edema, improve visual acuity, and reduce the shock waves of visual acuity A and B, and reduce the latency of CMT, visual acuity A and visual acuity B.
