摘要:

高位肛周脓肿是肛肠科的急危重症，后期容易形成高位复杂性肛瘘，给患者带来很大的痛苦。沙静涛教授认为本病的病机主要是湿热内蕴，气血阻滞，郁而化热，热盛肉腐，酝酿成脓。治疗上既行开窗旷置配合置管冲洗引流术以治其标；又从整体入手，辨证论治以治其本。提出以“托”贯穿始终、先清后补、分期论治的诊疗思路。术后早期采用“清托”法，方用仙方活命饮加减；中后期采用“补托”法，方用托里消毒散合补中益气汤加减。同时自拟湿敷坐浴方局部湿敷坐浴以清热燥湿、泻火解毒、消肿止痛、收敛生肌。沙静涛教授独到的治疗方法及经验，缩短了疗程，减少了术后并发症及后遗症，提高了临床一次性治愈率，为高位肛周脓肿的治疗提供了新思路。

Abstract:

High perianal abscess is a critical disease in anorectal department. It is easy to form high complex anal fistula in the later stage，which brings great pain to patients. Professor SHA Jingtao believes that the pathogenesis of this disease is mainly damp heat accumulation，Qi and blood block，depression and excessive heat，meat rot，brewing into pus. In the treatment，both fenestration and drainage were performed to treat the symptoms; From the whole point of view，we can treat the root of the disease by syndrome differentiation. This paper puts forward the idea of “supporting” throughout the whole process，clearing first and then supplementing，and treating by stages. In the early postoperative period，“Qingtuo” method was used，and Xianfang Huoming decoction was used; In the middle and late stage，the method of “Butuo” was used. At the same time，the self-made wet compress hip bath formula was used for local wet compress hip bath to clear away heat and dampness，relieve fire and toxin，relieve swelling and pain，and astringe and promote muscle growth. Professor SHA Jingtao’s unique treatment method and experience shorten the course of treatment，reduce the postoperative complications and sequelae，improve the clinical one-time cure rate，and provide a new idea for the treatment of high perianal abscess.High perianal abscess is a critical disease in anorectal department.
