摘要:
目的观察针刺联合Mckenzie疗法与单纯Mckenzie疗法对气滞血瘀型颈椎病患者颈痛及颈椎曲度(Borden值)影响的差异性?方法采用随机数字表将62例患者分为治疗组(针刺联合Mckenzie疗法)和对照组(Mckenzie疗法)各31例,其中每组各有2例脱落未被纳入观察?治疗组针刺腧穴取颈夹脊穴?大椎?天柱?天宗?膈俞?血海等,针刺结束后配合Mckenzie疗法,每周内连续治疗5次,周末休息,共治疗4周,对照组单纯采用Mckenzie疗法,疗程同治疗组,通过观察两组治疗前后NorthwickPark颈痛量表(NPQ)?视觉模拟评分(VAS)及颈椎曲度(Borden值)的变化,评定其临床疗效,记录不良反应?结果两组患者治疗后NPQ量表评分较治疗前均有明显下降,其中治疗组NPQ量表评分显著低于对照组(P<0.05);两组患者治疗后VAS量表评分较治疗前均有明显下降,其中治疗组VAS量表评分显著低于对照组(P<0.05);两组患者治疗后Borden值较治疗前均有明显升高,其中治疗组Borden值显著高于对照组?结论针刺联合Mckenzie疗法治疗气滞血瘀型颈椎病疗效确切,值得临床推广应用?
Abstract:
Objective To observe the differences between the effects of acupuncture combined with Mckenzie therapy and Mckenzie therapy alone on cervical spondylosis and cervical curvature ( Borden value) in patients with cervical spondylosis characterized by stagnation of qi and blood. Methods A random number table was used to divide 62 patients into a treatment group ( acupuncture combined with Mckenzie therapy) and a control group ( Mckenzie therapy) with 31 cases in each. Two cases in each group were not included in the observation. In the treatment group, acupoints on the neck Jiaji, Dazhui, Tianzhu, Tianzong, Geshu, Xuehai were used, and then combined with Mckenzie therapy for 5 consecutive treatments per week except weekend with a total of 4 weeks of treatment. The control group used Mckenzie therapy alone, and the treatment period was as same as that in the treatment group. The clinical effect was evaluated by observing the changes in the Northwick Park Neck Pain Scale ( NPQ) , Visual Analogue Scale ( VAS) and cervical curvature ( Borden value) in the two groups before and after treatment and recorded the adverse reactions. Results The scores of the NPQ of the two groups after treatment were significantly lower than those before the treatment, and the NPQ scores of the treatment group were significantly lower than those of the control group ( P <0. 05) ; the VAS scores of the two groups after treatment were higher than those before treatment. The VAS score in the treatment group was significantly lower than that of the control group ( P <0. 05) ; the Borden value of the two groups was significantly higher than that before the treatment, and the Borden value of the treatment group was significantly higher than that of the control group. Conclusion Acupuncture combined with Mckenzie therapy is effective in treating cervical spondylosis of stagnation of qi and blood stasis, and it is worthy of clinical application.
